Nick (Til Schweiger) is a janitor at a mental hospital, and runs away with an inmate Leila (Johanna Wokalek). They go into a men's bathroom together so that he can pee, and she tells him not to stand and pee because it will make a mess. (This is what they had been told in the hospital, because the janitors believed that mentally ill people often missed the bowl.) Two other men come in and are heard using the urinals while Nick and Leila are hiding in the stall, and Nick tries to sit but cannot pee that way. Finally the men leave and he pushes Leila out of the stall so that he can try to relieve himself. The scene ends at that point.
At Nick's brother's wedding, Leila announces that she needs to go to the loo.
Later, Leila is desperate to pee while waiting for Nick on a bench at a railroad station. She doesn't know that Nick has run off and left her, and she is shown crossing and uncrossing her legs in desperation. Finally a guard comes up to her and asks if everything is all right, and he lets her run off to use the toilet with her hands at her crotch.
